「安全態度問卷(Safety Attitude Questionnaire, SAQ)」為我國醫療機構普遍使用之調查工具,目的在於了解各級醫療機構臨床人員對安全態度的認知,是我國針對醫療專業人員少數的大型調查之一;確保大規模調查的品質與內涵,對我國整體醫療產業管理與確保病人安全,相當重要。
本研究採用台灣南部某醫學中心「病人安全文化調查」2014年至2017年4年調查結果之次級資料為主,研究對象為醫療、護理及行政人員等,並輔以單位離職率等客觀數據,除檢驗量表的適切性,並進行描述性統計、相關分析、跨層次模型與潛在成長曲線模型分析檢驗量表各構面的關係。
研究結果發現,8構面之「病人安全文化調查」量表品質大致良好;量表構面中「團隊合作」、「對管理的感受」、「工作狀況」,對「對工作滿意度」存在跨層次的正向關係;「對壓力的認知」、「工作生活平衡」對「對工作滿意度」存在正向關係;「復原力」為「對管理的感受」與「對工作滿意度」的跨層次正向關係以及「對壓力的認知」與「工作生活平衡」對「對工作滿意度」的正向關係之中介因子。
本研究除對8構面之「病人安全文化調查」之調查品質進行檢驗,並深入探究調查資料的構面內涵,搭配理論演繹,希望能透過確保調查工具品質、延展調查結果深度與廣度,使「病人安全文化調查」具有更準確的效用與內涵。
Abstract
Safety Attitude Questionnaire (SAQ) is the most popular scale for hospitals to measure safety attitude and climate in Taiwan. Organizations with good culture in patient safety are highly related to sound interaction among all members. Therefore, the purpose of this study is to examine the factors associated with employees’ job satisfaction and turnover rate, particularly the climate of patient safety.
This research used data of patient safety attitude survey collected by one medical center in southern Taiwan from 2014 to 2017. Study samples included physicians, nurses and administrative staff in the survey hospital. Turnover rates of different units were provided by department of human resource. The statistical analyses include describe statistics, correlation, factor analysis, CFA, multilevel model and latent growth curve model.
Results indicated that teamwork climate, perceptions of management and working conditions were correlated to job satisfaction positively with multilevel analysis. Stress recognition and work-life balance were correlated to job satisfaction positively. Emotional exhaustion mediated the relationship between multilevel correlation between perceptions of management and job satisfaction as well as the correlation between stress recognition and work-life balance to job satisfaction.
By ensuring the quality of the survey tools, this research deeply explored the relationship of dimensions and the connotations of the survey data with theoretical deductions.
