中文摘要
因為失智社區服務據點因區域人口結構與組織治理模式的差異,造成失智據點內部組織管理及外部社區協力上有所不同,因此提出三個問題,探討失智社區服務據點類型為何?失智社區服務據點組織管理現況為何?失智社區服務據點社區協力情形為何?
首先失智據點類型透過文獻探討及次級資料分析,針對六都各行政區總人口數、人口比例、人口密度、老年人口數、老年人口比例、扶老比、老化指數、及資源供需衡量指標進行組織外部分類維度選取,並依據組織構型理論進行組織內部維度選取,運用類型學進行失智據點類型分類。再者依據組織管理訪談大綱針對組織的服務對象、組織運作、財力及物力資源、人力資源及服務提供進行深度訪談;最後以社區協力訪談大綱針對初始條件、領導型態、制度設計及協力過程進行訪談,研究對象以高雄市21個曾經接受委託或現行持續辦理的失智社區服務據點之管理者為受訪者。本研究發現:
一、失智社區服務據點依據老化指數及治理模式可分為四種類型,包括高老化社區平面治理模式、高老化社區立體治理模式、老化社區平面治理模式、老化社區立體治理模式。
二、就各類型失智據點在組織管理方面進行異同比較,發現幾項焦點問題,包括(1)政府委外服務是否依服務對象的需求差異制定服務項目?(2)如何提升失智據點組織運作能力?(3)服務委外是否有效提供組織財力及物力支持?(4)政府服務委外契約如何解決人力資源的迷思?(5)服務委外績效是否應考量失智據點承辦組織價值性服務的提供?
三、另外就各類型失智據點在社區協力方面進行異同比較,包括幾項焦點問題,(1)初始條件不利於失智據點組織開展社區協力。(2)社區協力網絡缺乏促發者及領導者。(3)組織普遍缺乏制度設計的概念。(4)短暫性的社區協力該如何突破並維持運行。
最後,根據研究結果,本研究提出持續性的失智社區服務據點模型及失智服務網絡社區協力模式,此外不僅對政府及失智據點承辦組織提出相關建議,亦針對此一議題提出後續研究之建議及未來研究之可行性。
Abstract
Due to the differences in regional population structure and organizational governance models of dementia community service operators, the dementia community service operations are varuous. The study has three research questions: (1) What are the various types of community service in community-based dementia care centers? ;(2) What is the current status of organization and management of community-based dementia care centers? (3) What is the situation of community cooperation in community-operated dementia care centers?
First, the types of dementia bases are selected through literature research and secondary data analysis to select external dimensions, including the total population, population ratio, population density, elderly population, elderly population ratio, elderly support ratio, aging index, and resource supply and demand measurement indicators. According to the theory of organizational configuration, the internal dimension is selected, and the type of dementia base is classified by typology. Furthermore, in-depth interviews are conducted according to the organizational management interview outline, including clients, organizational operations, financial and material resources, participants and service provision. Finally, interviewers were instructed to explore their initial conditions, leadership types, system designs and cooperation processes based on the interview outline of the community cooperation. The research objects are 21 managers those who used to provided or are providing services in community-based dementia care centers in Kaohsiung city. The research findings are as follows:
I.Classification according to aging index and governance model. The four types of community-based dementia care centers are a three-dimensional governance model of elderly communities, the flat governance model of elderly communities, the three-dimensional governance model of aging communities, and the flat governance model of aging communities.
II.Comparing the similarities and differences of organizational management in various types of dementia bases, and found several key issues. (1) Does the government outsourcing service formulate service items according to the needs of client? (2) How to improve the organization’s operation ability of dementia bases? (3) Does the outsourcing service effectively provide financial and material support for the organization? (4) How does the government outsourcing service contract solve the myth of participants? (5) Should the performance of outsourcing service consider the value services of dementia bases?
III.In addition, the similarities and differences of community cooperation in various types of dementia bases are compared, including several key issues. (1) The initial conditions are not conducive to the organization of dementia bases to carry out community cooperation. (2) The community cooperation network lacks promoters and leaders. (3) Organizations generally lack the concept of institutional design. (4) How to break through and maintain the operation of temporary community cooperation?
Finally, according to the research results, this study proposes a continuous dementia community service base model and a dementia service network community cooperation model. In addition, it not only puts forward relevant suggestions to the government and the organizers of dementia bases, but also puts forward suggestions for follow-up research on this topic and the feasibility of future research.
