台灣因經濟進步和醫療品質提升，壽命延長及出生率降低，使得老年人口快速增加，在2018年3月已邁入高齡社會。爰此，自2010年起衛生福利部國民健康署開始推動「高齡友善健康照護機構認證」，其導入架構涵蓋：管理政策、資訊介入與溝通、友善環境、健康促進、社區服務與轉介等五大標準。該項政策並融入高齡友善的基本核心概念「活躍老化｣，活躍老化的三大基本組成架構分別為健康、安全與社會參與，運用其中來推行醫療機構的高齡友善政策，讓長者能在地老化、活躍老化、健康老化。
本研究採取質化個案研究之途徑，並運用以深度訪談、觀察法和次級資料分析等研究方法，以澎湖縣已通過認證的四家衛生所為研究個案，藉此瞭解澎湖縣基層衛生所如何推動高齡友善健康照護機構，以及探討政府面與民眾面對於高齡友善健康照顧機構之觀點比較。具體而言，本研究發現四所衛生所推動的模式有共同之處，但亦有針對個別衛生所情況因地制宜之調整。有關政府面與民眾面之觀點，基本上民眾對於政府推動高齡友善相關作為表示肯定，但本研究亦發現有些政府作為民眾並未察覺或者感到有需要改進之處。同時，本研究亦發現，基層衛生所承辦人員推動時會遇到許多政策、資源和實務上的困境。針對這些研究發現本研究研提相關建議，期望能提供爾後推動高齡友善健康照護機構之參考依據。

With the growing economy, medical care improvement, longer life expectancy and lower birth rate, Taiwan’s population of elders is growing rapidly. Taiwan has become an aged society in March, 2018. Therefore, since 2010, Health Promotion Administration of the Ministry of Health and Welfare has promoted a policy to certificate age-friendly hospitals and health services. The structure of this certification includes managerial measures, information intervention and communication, friendly environment, health promotion, community service and referral. Also, the policy incorporates the basic concept of age-friendly—active aging. Three major components of active aging are health, safety, and social participation.
This study employs the qualitative case study method and collects data from observation, in-depth interview, and secondary data. By analyzing four certificated health centers in Penghu, this study examines how local health centers promote the policy of age-friendly hospitals and health services, as well as the comparison between government and citizens’ perspectives. In specific, this study finds that these health centers have several common modes of promotion, but they may also adjust their measures based on local conditions. As for the comparison between government and citizens’ perspectives, most citizens hold a positive attitude toward the government’s related measures to promote age-friendly. However, this study also identifies some measures which are not recognized by citizens or should be improved. Besides, this study points out various implementation challenges for local health centers’ bureaucrats. This study concludes with several practical suggestions.
