美國醫學研究院提出跨領域團隊合作能力,為醫護人員需擁有的五大核心能力之一。跨領域團隊合作是由一群不同醫療專業人員互相的合作,共同目標為提高病人照護的品質和結果,並減少住院天數、臨床錯誤和死亡率。由於護理人員是日常疾病照護的關鍵成員,他們具有什麼樣的特徵影響跨領域團隊合作,是需要進一步探索的關鍵研究和實踐問題。研究目的為利用量化研究來探討重要個人特徵之護理師對跨領域團隊執行疾病照護之合作能力程度之差異,以及利用質性研究來探討護理師對跨領域團隊執行疾病照護之認知和其挑戰。
本研究採用兩階段的混合性研究法,以問卷為主,訪談為輔的方式獲得真實的資料。研究對象皆為年滿20歲之護理師,問卷共收回444份有效問卷,並利用Stata 14.0版進行問卷分析,包含獨立樣本T檢定、單因子變異數分析、多元線性迴歸;質性研究以深度訪談法進行,共訪談4位護理師,使用主題分析作為內文的整理及呈現方式。研究結果發現不同服務科室、擔任管理職位、有較高層級的護理師以及接受過跨領域團隊合作教育訓練呈現更高水平的跨領域團隊合作的表現。經深度訪談後歸納出10大主題及31個次主題,受訪者對跨領域團隊合作有著很好的理解,團隊合作對於提供有效的跨領域團隊合作照護是必要的,定期舉行跨領域團隊會議,並制定對策,藉此提供以病人為中心的照護。在跨領域團隊合作中會遇到許多挑戰,包含不當的溝通方式、以及因人力困難而造成照護不連續的問題等,因此教育訓練及建立良好的訓練機制很重要,才能維持成功的跨領域團隊照護。
Abstract
The Institute of Medicine (IOM) has proposed that interdisciplinary teamwork is one of the five core competencies that healthcare professionals should possess. Interdisciplinary teamwork involves collaboration among healthcare professionals from different disciplines, with the shared goal of improving the quality and outcomes of patient care while reducing hospitalization, clinical errors, and mortality rates. Given that nurses are critical members of the healthcare team in the daily care of patients, it is essential to explore the characteristics that influence their participation in interdisciplinary teamwork, which is a key area of research and practice.
This study aims to investigate the differences in the level of collaborative competence among nurses in executing interdisciplinary team-based disease care based on important individual characteristics, using a quantitative research approach. Additionally, a qualitative research approach will be employed to explore nurses' perceptions and challenges related to executing interdisciplinary team-based disease care. The study adopts a two-phase mixed-methods design, with the questionnaire as the primary data collection tool supplemented by interviews to obtain in-depth information. The participants include registered nurses aged 20 and above. A total of 444 valid questionnaires were collected and analyzed using Stata 14.0, including T-tests, one-way ANOVA, and multiple linear regression. Qualitative data collected through in-depth interviews with four nurses were analyzed using thematic analysis to identify ten major themes and 31 sub-themes.
The study results reveal that nurses working in different departments, holding managerial positions, having higher nursing levels, and receiving interdisciplinary teamwork education and training demonstrate higher levels of interdisciplinary teamwork performance. The in-depth interviews shed light on the participants' understanding of interdisciplinary teamwork, emphasizing the necessity of effective interdisciplinary teamwork to provide patient-centered care. Regular interdisciplinary team meetings and the development of strategies were identified as crucial for achieving successful interdisciplinary team care. However, the study also identified challenges in interdisciplinary teamwork, such as ineffective communication and issues with care continuity due to staffing difficulties. Therefore, education and training, as well as the establishment of a robust training mechanism, are crucial to maintaining successful interdisciplinary care.
